Roof Access Request, Vendor Registration & Release of Liability

To help protect Association property and improve safety, all vendors and contractors requiring access to the roof must complete this form before access will be granted.

Please complete all required fields accurately. Once your submission has been reviewed and approved by Management, a temporary roof access code will be issued for your scheduled service appointment. The access code will be active only during the approved time period and will automatically expire.

Completion of this form does not constitute authorization to access the roof. Access is permitted only after approval by Association Management.
